Institutional rate: US $400.00 plus postage and handling.
Individual rate: US $160.00 plus postage and handling.
Rate for individual AMS, SIAM, and MAA members: US $128.00 plus postage and handling.
Postage and handling: USA: $10.00. Canada: $17.50. Other countries (airmail): $25.00 per volume.
Back issues are also available for $105.00 each.
